Patient death: trust rebuked for ignoring warnings of senior nurse
A coroner has told East London NHS Foundation Trust it must make urgent changes after directors ignored warnings from a senior nurse about a colleague among other trust failings that helped contribute to a patient's death. The woman died after being admitted to a psychiatric ward in June 2022
