Nurse died by suicide after ‘extremely traumatic’ incident at work

A mental health nurse who had been experiencing a period of mental ill health died by suicide in March 2025 after witnessing a traumatic incident at work. In a prevention of future deaths (PFD) report, the assistant coroner highlighted a gap in the pathway questions used by 999 call handlers.
